Referral Assistance
Once the Plan of Care is in place, Senior Support Services will refer it’s client to resources within the community. These services are those that the client has decided will work best and will allow as much independence as possible.

Coordination
Senior Support Services will work with you as well as medical professionals to help coordinate the commencement of services.

Monitoring Services
Once services begin, we will be available to monitor services and ensure that quality care is maintained.
